How to Apply:
1. Download both the Application and Personal Financial Statement.
2. Complete and return along with the following documents:
- Financial Statements and Business Tax Returns for the most recent two (2) years
- Current Detailed Accounts Receivable Aging
- Accounts Payable Schedule
- Articles of Incorporation
- Sample Invoice and Backup to Invoice
- Business Plan or Resumes (if available)
